QR codes are ideal for:
Parcel inserts: Include a QR code on packaging inserts or shipping labels so customers can scan right after unboxing.
In-store displays: Place printed QR codes near checkout counters or fitting rooms so customers can review their in-store experience while it's fresh.
Handouts: Provide business cards or flyers with a QR code during events, trade shows, or pop-up markets to encourage follow-up reviews.

Review title: a short headline for the review (optional)
Write a review: the main review content
Name: their name
Email address: their email address, which will be used for verification and not displayed publicly
The Publish review as dropdown for the customer choose how chooses how their name will appear (John Smith, John S, John, J.S., Anonymous)
The "How we use your data: We’ll only contact you about the review you left, and only if necessary. By submitting your review, you agree to Judge.me’s terms, privacy and content policies." policy text
The Submit Review ➔ button to move to the next form
The media form to add a photo or video to their review, which includes:

Considerations
QR codes are public by design. That means anyone who scans the QR code can submit a review, even if they haven’t purchased the product. Web review restriction does not apply to QR codes.
It's not possible to customize the texts in the QR code landing pages, but you can adjust the form styling and submission flow as part of the Write a review form.
